You may contact Office of the Registrar Of Societies and obtain all necessary forms. You will have to file along with your application for registration MOA, Rules and Regulations of your Society along with list of the Members of the Managing Committee/ Trustees etc. Certain declarations and consents have to be filed by the President of your Society and also the certified copies of the Resolutions passed by the General Body of your Society adopting the MOA and the Rules etc.
