If he was a Govt. teacher, then his service book will be available with the Govt. In that service book, his date of birth is recorded. Get hold of that copy duly certified/attested. That should do. Or the official memo. retiring him from service. Or his Pension Payment Order etc. could be handy.

You asked whether sworn affidavit can be given as proof of date of birth. Please indicate whether such an affidavit is mentioned in the list of birh proof certificates at passport seva website.?

From the year of passing of matriculation and other records from pension file you can apply for the duplicate matriculation certificate and school leaving certificate. Being a retired government employee he must be having PAN no. in which date of birth is available. From the Govt. record, his service file can be traced and certified copy of the copy of certificate may be obtained.
