how the name came to be wrongly recorded.
what name you wrote in application,
what school forwarded
what was there in the hall ticket.
when did the error occur
when did you oppose it.
what the board officials advised you.

Foe passport following are few lines from passport office web site:
Q: Have you ever changed your name?>
If you have ever changed your name, put a cross in the Yes box and provide the details in Column 2 of the Supplementary Form For example, if you have changed your name from Vidhi Mehta to Aditi Mehta put a cross in Yes box. This will be applicable to an applicant who has even marginally changed the name or a female who has changed her name or surname after marriage. For example, if you have changed your name from Harvinder to Harjinder or Ritesh to Reetesh put a cross in the Yes box. If you have not changed your name ever, put a cross in the No box.

1. As regards the mispelt name of yours leaving one alphabet from VENKATA TO VENKTA there are two options to you.
A) Since it is SSC certificate which is the first proof of your name and date of birth, you can continue writing your name in all future applications either examinations, pass port applications or any other purpose as VENKTA. Hereafter there should be uniformity in your name. This is the easiest way of solving the problem.
2. If you wish to correct your name to VENKATA, you have go through your School where from you forwarded your examination application. You have to apply to the Secretary Board of Secondary Education stating that an error is crept in the certificate The school authorities will certify the correct name as entered in the examination application and forward it to District Educational Officer. The DEO will forward the application to the Secretary, SSC Board. You have to pursue the matter with the Board of Secondary Education for carrying out correction. This is a lengthy procedure.
2. As regards your father name the full name will not appear in the Computer generated certificate. Hence you can conveniently ignore as there is no error except abbreviation and short cut.
Decide what course of action is to be adapted.
