fraud with patient

sir my self alok pandey advocate and i started my practice from august-2010 and now i have a case that :- my friend"s wife is pregnant from 10 weeks and when she went for ultrasound Dr. checked her and gave the report that her baby has died in her stomach and made the report and she was totally shocked after hearing it and after few days when she went to another lab for confirming the report they told that her baby is alive , and first doctor gave her false report so now my friend want to file case and claim from that doctor who give them false report so plz tell me first step which should i take against the doctor plz ------ sir give me advice

sir should i file the complaint before my district consumer forum on a simple paper and plz tell me section and act which i should mention in the letter and should i have to attach doctor report copy with the letter

you have to file complaint in duplicate with name and address of of both of parties on it and telling the facts, grievance and relief sought say penalty / compensation etc. and also attach the copy of all documents you have and mention about them in complaint also making them as annexures to the complaint. amitminocha@yahoo.com

sir i have a problem , in the altrasound report it is usually written on the bottom of the report that " this is a professional opinion not a diagnosis, it needs clinical correlation, report is not valid for medio legal purpose" sir in applicant wife report it is also written , now sir what should i do , now can i claim on diagnosis center for their doubtful report , sir plz give me any contact number of urs so that whenever i need any help i can contact to u , i shall be thankful to u
