How to change mothers name in child birth certificate

The mothers name in my child birth certificate is "RITA" and i want it to change it with "POONAM".I do not have the documents in the name of RITA.
Please let me know the procedure of changing it legally?

Consult birth certificate issuing office , find out the procedure for rectification of the error, complete the formalities.

How was a different name registered with the registrar's of births and deaths at the time of registering the birth and name of the child ?, It may not be possible to change the name in the birth register and get a fresh birth certificate now, only an affidavit stating that both the names are one and the same may solve the purpose.

Sir! i want to amend it in the same birth certificate which has "RITA" as mother.
What is the legal procedure of changing it to "POONAM" instead of "RITA"? Please let me know the procedure if any?
