Is annexure d required in case of a fresh passport appn for a married woman?

Hi all,
I want to apply for my mother's passport online. Is it necessary to submit Annexure D (Affidavit for Lady applicant for change of name after Marriage) along with the other documents (Address proof, DOB proof, docs required for Non-ECR)?
This one will be a fresh application since she did not have a passport before. However, some of her documents (birth certificates, Matriculation certificate, etc) have her maiden name written on them.
Please help.

It shall be better if the Annexure D is also enclosed apart from the fact that her earlier certificates do not contain sirname attained to her after her marriage.
