Legal procedure for adding surname with first name

I would like to know about the legal procedures if i wish to add surname with my first name.
I wish to use my full name legally for all future documentation and future purposes.
I request you to please guide me.

Publish a notice in news papers regarding change of name.

Dear Rajesh,
I don't know the procedure in your state. But in Tamilnadu, you have to effect the name change by Gazette Notification to published by the state government. In addition to that you have to publish the notice of change of name in two newspapers (english and regional language).
I suggest you to check the rules for change of name in your state.

Apart from Krishna remarks i would like to add that made an affadavit and file a declaration suit after that gazette and publishment of change in name will be more concerete.

Could you tell me how to file declaration suit and where i can file the same?
What are the documents i have to be submitted at the time of filling?
Filling must be after publishing notification in Gazette?
Any help would be appreciated.

The declaration suit would be filed in the court having jurisdiction of your area after newspaper publication and gazette notification.

I dont know that you have discussed many times.
And i also used google before coming this forum. But, i didnt get appropriate approach. Thus i am approaching this forum to get better opinion.
