Letter format for cancellation profession tax no

Sir, Profession tax Department allotted Two PTRC no our company. One is from Bandra Office & another is from Nariman Point office. We have paid profession tax as per Bandra office no. Now Nariman point profession tax department send us notice for show case in default filling & payment. Now we want to cancel PTRC no from Nariman Point PTRC no so kindly help me how to draft letter to Profession tax department
