I think you have filed your written statement. If have filed then file an application to amend it adding the removal of name from health policy as one of the grounds showing cruelty on the part of petitioner against respondent. You can accordingly get amended your application if filed and not decided application under section 24 of HMA.
There is limitation of family court in case of divorce petition. It has to decide the matter as per relevant provision which do not contain any mechanism to direct the petitioner to include your name in the health card or provision of refunding you the expenses to be incurred on the treatment of your daughter.
Clearly speaking, you shall either have to file a maintenance petition under section 125 criminal procedure code for you as well for your daughter wherein you can demand maintenance against each and every head which you were enjoying while everything was right and accurate. You both are entitled to avail the similar life style and status as is being availed by your husband under this petition. so you shall have to file this petition before the same family court.
The respondent in that petition (your husband) can also be directed the court either to include your name in the aforesaid health coverage scheme of pay/reimburse each and every expenses connected thereto.

DV Act petition under section 12 can also be filed against your husband and can additionally be demanded the due compensation and regular maintenance in addition to petition decided under section 125 criminal procedure code.
If bills are paid by you, you only would have the bills. He can not submit to receive the amount, hence you can give him the bills against money.
