Mistake in gazette name change

sir, madam i 36 years ,reside in tiruppur of tamilnadu, recently in chaned my name from v.karthik raj to p.v.karthik raj as per tamilnadu gazette. P is first letter of my grand father and V is first letter of my father name.

Now, there was a column called "native place" in the name change application gazette where we have to write our city of birth as per the instructions overleaf. but i wrote my place of living(tiruppur) in that coloumn and got published. but my birth place was coimbatore(hospital) and city of living is tiruppur from my birth. how i can correct this error.

And second error in that same gazette publication is that they have omitted to publish the word "extension" in my address

sir, please advise how to correct these errors, because i own three properties in my old name and i should not have to face any problem because of these errors is future.
