You submit application with affidavit.

The change of name in certificates would require affidavit and proclaimation.

Thereafter you would need to get it changed in all documents.

For Banking only a application by the concerned lady is enough.

For social reasons, no need of any affidavit or anything. Just change the name.

Regards,

Legal formalities required for any name change is to publish the change so desired in official gazette as well as in two news papers one of national circulation, next being regional, and then applying with all those authorities where ever the old name exists in records to substitute the older with newer one as published in the official gazette.

you have to publish the name in official gazette as well as in two leading daily newspapers.
