Name change in odisha

Hi,
I want to change my name. Can anyone explain what is the legal procedure for the same. I am resident of odisha but currently working in Bangalore. I have already made an affidavit regarding change of my name and published in one of the leading local newspaper in odisha. I also have made a draft of Rs. 770 and have 2 copies of Form 10 (Specimen of name change) issued by Dept of Publication, New Delhi. But i am not sure what is the next procedure.
I have couple of questions which i have mentioned below.
1. I also dont know to which dept in odisha eed to apply ? or do i need to apply in dept of publication, New Delhi ?
2. Do i need to submit my original affidavit to Dept of publication ?
3. I have published in only newspaper since Dept of Publication guidelines specifically does not mention to publish in two newspaper. Is it OK or do i need to publish it in two newspaper ?
Can anyone please explain regarding the above queries. I will be happy if any expert gives his/her contact details like mail id and phone no.

The change of name requires notification in the Gazette of India Part IV. The following is the procedure addition for change of name, addition or deletion of surname etc
The person attaining the age of 18 yrs. and above who wishes to get his/her change of name published in the Gazette of India Part-IV has to comply with the following formalities prior to publish of such advertisement.
1. Advertise change of name in one of the daily local leading newspaper stating therein father’s/Husband’s name along with residential address and forward it in original to the controller of Publication, Department of Publication, Civil Lines, Delhi-54.
2. Submit an original affidavit duly attested by first class Magistrate/Notary/Oath Commissioner showing therein full details of father’s/husband’s name along with residential address before taking oath in the affidavit . In case of individuals working with Central Govt. they may submit deed for change of name in accordance with the Ministry of Home Affairs O.M. No. - 190016/1/87-Estt. dated 12.03.1987.
3. Submit printing matter as prescribed in the specimen duly completed in all respects by typing the same on a separate plain paper and signed by the individual in his/her old name, with two witnesses in duplicate. The Proforma must be typed.
4. Attach two passport size photographs.
5. The prescribed printing charges for publication of change of name has to be paid to the Controller of Publications, Department of Publication. The amount will have to be submitted or remitted in advance in cash or by Demand Draft/India Postal Order drawn in favour of Controller of Publication, Department of Publication, Civil Lines, Delhi-54. The extra copies of the Gazette can be supplied to the extent of 2 on additional payment. The charges in cash can also be deposited alongwith, aforesaid documents in our Department. 6. All the aforesaid document, printing charges together with forwarding letter to be sent the controller of Publication, Department of Publication, Civil Lines, Delhi-54. The documents must not be old beyond the period of one year.

Hi Mr. Jaggarao,
Thank you for your quick response. However i just want to know since i am a resident of odisha, can i apply to Controller of Publication, Dept of Publication, Civil Lines, New Delhi-54 for Gazette Notification ? Or do i need to apply in any odisha govt dept for Gazzette Notification ? Can you clarify on this. If i need to apply in odisha then which dept i shall apply ?

You can apply to New Delhi for Gazette Notification. That is sufficient.

Hi Mr. Jaggarao,
Thanks for your advice. It will be very very useful for me. Just proceeding with some more queries from my side. I want to know after changing my name legally will i be able to get my educational qualification change with my new name ? Will the educational certificates be changed by the respective boards ? I believe however i can get new voter id, passport, PAN card etc through the respective govt. depts. But i am not sure about educational qualifications. Can you please advice on this.
