File a case for getting court order, if it is necessary to change the name.

If an affidavit mentioning the both the names belong to one and the same person will suffice, make it or file a declaratory suit before a civil court declaring the name as so and so and by the order o the court, you may correct the Municipal records of birth registration of your son and get a fresh birth certificate accordingly.
