These form can be attested by any Gazetted officer or by branch manager of nationalized bank or by post master of post office, and can be directly submitted to epf office after attestation.
otherwise go to the link and complaint to epf commissioner-

The employer has to attest the forms and submit it to concerned PF office within 5 days and supply the acknowledgment issued by PF office to employee.
If you have POD attach it and submit a representation to appointing authority, MD and still if they don’t apologize and supply the acknowledgment issued by PF office you may lodge a complaint ( in person) in writing under acknowledgment with RPFC in any nearest PF office.

You can submit PF forms at any nearest PF office.
Meet RPFC in person.
However you may draft a letter addressed to RPFC and explain the submission of PF forms (attach POD) and conclude that employer and its authorized signatory in HR has not attested the PF forms and has not submitted to PF office and has not supplied you the acknowledgment issued by PF office (for you) to you despite repeated representations (narrate all even if by phone, emails, in person , by letter thru post) and request the RPFC to initiate action against the employer and its authorized signatory in HR as refusal to attest the PF forms is cognizable offence and also help you to submit your PF forms attested by BM of the bank.
Obtain acknowledgment with diary number and seal and signature from RPFC.
Demand reply by redg. post only.
It is suggested that you may refrain from transacting by email, phone etc.
It is always better to keep the PF /ac regular for at least 10Y and be eligible for pension.
There are many other benefits of keeping PF a/c regular that you can see at
www.epfindia.gov.in
and learn from RPFC.
Therefore if suitable you may rather apply for transfer of PF.
Your complaint on declinature to attest PF forms shall still be valid and shall stand good for action by EPFO.
Let the EPFO take a stern action and thus reduce one offender HR person and one employer from society.

Well advised by the expert Kumar Doab ji. Lodge complaint and claim with EPFO as advised. employer can not refuse attestation of claim form.

Many of such companies are gone with the wind. Before they disappear act immediately.
