NOMINATION UNDER MAH COOP SOC ACT

My brother and me are having equal share in a flat located at Mahim, Mumbai. This flat was originally owned by our uncle who expired 3 years ago and was transferred to our joint names in equal share as per nomination made with the society.
Both our names appear on the share certificate. I am married with 2 children but my brother is unmarried.
I wish to nominate my share of 50% to my wife. My brother is also willing to nominate his share of 50% in favour of my wife.
Are we required to fill in separate nomination forms ie Form No 14 under Bye law No. 32 or a single form has to be filled. Please advice

yes my brother is not married and he is also willing to nominate his share of 50% in favour of my wife ie his sister in law.
