Obtaining hospital records of a patient

A patient died in a hospital and there are reasons to believe that some negligence has taken place during the treatment.It is proposed to consult a medico legal advocate for taking up the case. How to get the hospital documents with regard to the patient with out being tampered.

On discharge the patient is entitled all documents related to his treatment including Bed Head Ticket.
The non supply of it would make the hospital more liable for deficient of service.

Ask from the hospital, if not provided on request send registered letter. If Government hospital, RTI avenue may also be adopted.

Obtain all original diagnostic reports too and examine the patient file.....and obtain discharge summary.
