i want to file the public interest litigation without advocate in the high court of andhra pradesh . but i dont know the proforma to file kindly send the proforma to my mail , then i will alter it for my case convinience and then file in high court of AP, kindly do the needful

There is no particular proforma for filing of the PIL. You may address it in a letter form also with a request to treat the letter as PIL provided the subject is of that importance. If the facts are not enough to constitute public interest litigation, you will have to pay costs. So take care.

to file the public interest litigation, the unsolved problem should be related with public at large.
you can file case under public interest litigation by writing a letter, stating the detailed facts, to the chief justice of high court.
there is no specific proforma for PIL.
