Shop & establishment registration' in delhi

'Shop & Establishment Registration' in Delhi. ( http://www.labour.delhigovt.nic.in/ser/)
I've registered my small startup computer related business through a agency on 13-Jan-15. They have provided a 1 page certificate 'Form C' (computer generated).
My question :
1. Is this the only certificate we get?
2. If i need to re-generate the same where is the option (link?)
3. I can find my detail under option 2. (please refer the attached image) but unable to locate under option 3.
Option 3 says if the Cerficate is issued (in the last column)
Is my business is really registered as it is not showing up on option 3 list?
Please help me, unable to start my regular work without the papers.

Show the certificate of registeration to a local lawyer or get it authenticated and verified from the issuing authority.

You have stated to have taken the help of an agency o set u this organisation, did you not clarify these details from them?, what was their response? The facts and materials are in your possession, so it is always better to give an opinion after viewing them, therefore you may clarify from them itself (since you have paid them for the services) or from the office directly where you are going to submit them.
