Dear/Sir(s),
How a person can change his name?The facts of the case are that a female chide was born before 16 years on x date.A wrong entery of name of mother was recorded at the time of birth of that child.The death and birth registrar also recorded her name wrong.what is the procee of her name change?

Apply for Certificate as it is recorded . Then apply for corrections in the entries and if necessary an order may be rquired from Reveneu Divisoal Officer for correction of the entry as it pertainign to 16 years ago , it can be co related with facts and with other Certificates and evidnece of Hsopital rcords etc !

Sec 15 of the Registration of the Births ands Deaths Act 1969 - "Correction or cancellation of entry in the Register of Births and Deaths" Procedure prescribed -Rules as framed by the Concenrend State Govt .
