Use of alias in name

Name of wife as per school records is Parveen Kunari.After marriage it has been changed to Praveen Sharma with an adapted nick name Neelu. Can I use this name as alias to Praveen Sharma. If so, then pkease advise me the procedure.

you may go govt gazette publication office, they will give name modification form, fulfill the application and issue the publication thereafter you can use as per your wish.
