Supplementary rental agreement

Hello, 

The issue - We are four members living in a flat, and one of us is moving out and will be replaced by other individual in his place, and there is a leave and license agreement in the name of the 4 currently staying. 

Question - Can a supplementary agreement be created mentioning details that the person moving out will be replaced with a new entrant and whether the same can be registered online. ??

If yes, kindly provide a draft for the same.
