draft a adoption deed and then apply to the SR office where you reside or the donor parents reside in any jurisdiction along with the baby pic and baby shall be carried to the concerned SR office.

It would be better to seek legal service of a local lawyer for proper drafting, presentation and registeration of Adoption deed in a professional manner.

As per custom adoptions are made by performing the rituals and executing the deed of adoption which may be registered with the Sub Registrar of assurances. In the deed of adoption the consent of the natural parents to give the child in adoption to u has to be mentioned and the natural parents should sign the deed of adoption. Of course performing the rituals is optional.
