First you have to contact any local lawyer.
The procedure is
First you have to submit the application to Taluk Office or Panchyat Office and also submit the application to sub registrar office.
In both the office, you will get the non availability certificate.
Based on the non availability certificate, your lawyer will file a case in the court.
After all the enquiry & paper publication, the court will order your local Taluk Office or Panchyat Office to issue the birth certificate.
it will take 30 to 45 days.

Whether you are having School leaving certificate, matriculation certificate, Hospital record, PAN card or any other proof regarding your birth date?
If your state is having procedure of entering DOB on late information (some states are having), You should apply to the competent authority for registering your birth date and pay the fee for late information. The competent authority would order the registrar to register the DOB and you would get the certificate.
In chennai the procedure is as under:(refer the Site of Chennai Corporation)
With a late fee of Rs.10/- & with orders of Judicial Magistrate or a Metropolitan Magistrate - after one year.
