addressing a HC judge

Please enlighten me the most appropriate form of addressing a HC/SC judge. What niceties should be borne in mind while pleading before a HC/SC judge?

Are lady judges to be referred to as 'My Lady' instead of 'My Lord'?

Information

A request to seniors to guide me on the belowmentioned two issues:

First: Whether it is mandatory for an employee to give three months' prior notice to the employer before leaving the job? And secondly, if the employee gives only one month's notice and leaves the job, whether the employer has the right to deduct three months' salary from the dues which he is entitled to once he ceases to be the employee after resignation? (Note: In this case, the employer relieved the employee after one month's notice period but deducted three months' salary. Can he do it as per the law?

Appeal under Arbitration Act

Section 37 of the Arbitration and Conciliation Act deals with Appeals.... "appeal shall lie from the following orders (and from no others) to the "court authorised by law" to hear appeals. "court" has been defined under section 2 as follows - 'court' means the principal civil court of original jurisdiction in a district, and includes the High Court in exercise of its ordinary original civil jurisdiction....does it mean that appeals can lie even in district courts or one has to necessarily approach the high court?
