Change in Name after marriage

Dear Sir,
I have got married before 4 months, i want to renew my present ESI card as my family is changed now. Please tell me its procedure, i am in pune. (my previous ESI card is misplaced, before marriage) Please tell for New ESI card procedure. After marriage my name is changed, but tell me how can i change it by rule and its procedure. Someone told me i have to apply to Gazzeted Officer regarding this matter. Please help me.

Make an affidavit on a stamp paper of Rs. 10 declaring that from now you will be known as the changed name, then make it duly notarized and then make it published in a newspaper.

Though your queries has been rightly answered by Mr. Sanjay Dixit but apart from the Affidavit you are supposed to publish the same in National News Paper daily. This will complete the basic formalities needed for your purpose.

Dear Priya,
Mr Dixit is correct but it is better if you can send a copy of said affidavit to the state Gazettte of the respective Government for publication along with the required fees than it will be better and will act for all the purposes.

Mr VV Ramdas correctly added a last step in this procedure. Before that Mr. Dixit, Mr Thakur hinted on a procedure for change of name is right.

u will have to make an affidavit on Rs.20 stamp paper and get notarised it and then publish in the news paper regarding the cange in nme

I agree with the above views of my Learned Friends, and I also suggest you to produce the True Copy of Marriage Certificate before the ESI, and give an application for change of name and re-issue of new ESI Card.
