Help in filling annexure g

(Querist) 09 December 2015 This query is : Resolved 
my son is 16. Applying for passport . Parents don't have passport . His father and I are separated yet not legally divorced. He is out of contact for several years. I don't have the marriage certificate . I have 3 queries
1)where should I get it Notarized and how ? I'm from Vizag. Should I go to court or something. Is it hard to get it Notarized ? Should I show marriage certificate ?
2)what date should I provide in the annexure? Like today or the day I want to get it Notarized , or anyday ?
3)what would be an apt reason to provide in the reason blank. So I will have least trouble .

There is Passport Office in Vizag itself. There are a number of notaries adjacent to the Office.

You should visit passport office and discuss there.

Document can be notarized easily. Better have date of the date of notarization.

Fact should be mentioned to have least trouble in future.

Find the necessary requirements from the PRO of the Passport Office at Vizag.

Do not approach a notary sitting near the passport office, they will charge exorbitant fees. Their charges will be ten times more than the regular notaries having office elsewhere in the city. The notaries having office near passport office will capitalising the urgency and helplessness of the people approaching for attestation for passport purposes as their weakness.
You can patiently read the contents of the affidavit required for passport get them typed in the appropriate non-judicial stamp paper, get it notarised locally submit it to the passport office along with the passport application. The nominal charges shall be Rs. 100/- or little more by the a notary for attesting the affidavit.
