Mr. Mehrotra has had an impeccable record of undertaking challenges and working diligently. He was however superseded by a person two years his junior when it came to becoming the Head of his section. Agonised by this decision, Mr Mehrotra filed an RTI application with the PIO of his organisation to know why he was not in contention for the promotion and what were the notings by his superiors in his ACRs.
1. Are file notings pertaining ACR open to Mr. Mehrotra? Do any conditions apply in this regard? If yes, state them.
2. Is the person who superseded Mr. Mehrotra a Third Party? Give reasons for your answer.

by rti you will not get answer to any "why" questions.
You can collect available information,including details of acr and use that "information" for your purpose.

There is a recruitment rule for every such situation. One has to first know whether the promotion is based on serniority or seniority cum fitness, or selection.
Further more, what is the minimum bench mark required - whether it is Good, Very Good, Outstanding etc. etc.
Without knowing these details it is not possible to give a view.
It looks the questions raised by you are relating to RTI Online Certificate Course examination. If it is true, it is better write answers honestly by yourself.
