Gratuity eligibility

Hi,
I have a query on gratuity . I have worked 4 yesrs 9 months in a company of 6 working days per week . The duration was 17th Sepetember 2008 to 25th june 2013( total 4years 9months 8 days). Company was limited organization.
Could you please tell me wheather i am eligible or not ?

I approached my Previous company HR person ,What they are saying that they are linked to LIC related to Gratuity Payment. So as per their policies you are not eligible. Please clarify I am eligible or Not.

Submit FormI under proper acknowledgment and mention in your draft as suitable to you covering that ......... 'Notice of Determination of Gratuity' and payment of Gratuity has not been supplied to you despite representations made to Mr/Ms..............in person/by phone/emails etc etc and FormI is attached.
