It's not a very elaborate document - it just have to evidence that fact of giving and taking of the child with full knowledge of its implications and the fact that child is hereby implanted in adoptive family with full rights of a natural born child.
Get it registered for added sanctity and presumption as to correctness
ajay sethi (Expert) 05 March 2012
adoption must be by a deed in writing duly regsitered . if natural parents are giving child fro adoption then the signatures of natural parents are a must . all ingredients of Sections 6, 11 and 16 of the Hindu Adoption and Maintenance Act, 1956 must be complied with.
ajay sethi (Expert) 05 March 2012
Section 6 of the Act lays down conditions for an adoption to be valid. One of the necessary requisite for a valid adoption is the person giving In adoption must have capacity to do so.

Section 9, which is about the person who are capable to give in adoption. It provides that no person except the father or mother or the guardian of a child shall have capacity to give in adoption.

Presumption as to registered documents relating to adoption. --Whenever any document registered under any law for the time being in force is produced before any Court purporting to record an adoption made and is signed by the person giving and the person taking the child in adoption, the Court shall presume that the adoption had been made in compliance with the provisions of this Act unless and until It is disproved."

No room to put my opinion because mr sethi filled already
Adv.R.P.Chugh (Expert) 05 March 2012
Let me correct Mr.Sethi when he says it is required to be registered - it is not - however it is advisable to do so.
Raj Kumar Makkad (Expert) 06 March 2012
I have nothing to comment except that the will can be oral also but this is advisable to get it registered.
