Format of application for permission to appear and argue in person

I have filed PIL in SC, same diary no. is 37414/2013.
Now SC asking me that file "AN APPLICATION FOR PERMISSION TO APPEAR AND ARGUE IN PERSON"
Therefore I kindly request to you, that please give me format of same application.
Please.

Dear Nadeem Ji,
Can you suggest proper name of the same Drafting book where I can found same details.
Is it possible without any format, I can write same "APPLICATION FOR PERMISSION TO APPEAR AND ARGUE IN PERSON" in my words.
What will you think can SC accept my same application if I write in my word.
Do you know any official or unofficial web link where all kind of forms format available regarding SC. I have gone through Handbook and rule of SC but I don't find same.

PL DON'T REQUEST LAW EXPERTS TO DO YOUR CLERICAL WORK..
Any ordinary Adv can get it for u. Why u need experts for it??
Do u know diff of Specialists Consulting Dr. & local practice Dr. in ur street? ??

I am very sorry, But this is legal problem to me in the view of PIL, and there is no parson in my contact who has SC experience and who will provide me free legal advice.
I have dicided to go with common way with people problem.
Hope if you have any out puts than provide me.

I do not think there is any specific format for the purpose. You may however seek guidance from the SC Registrar's Office.

I agree with the advice of Expert of Mr.Gupta. There is no specific format as I recently in AP High Court.
You can draft a simple affidavit in your own words and name as
"APPLICATION FOR PERMISSION TO APPEAR AND ARGUE IN PERSON".
This will suffice.
