In my 10th certificate my name is written as Brijesh.
My father use his name as R R Chaudhary
But my surname is 'Kasana'. In banks , LIC Policy,PAN card my automatically they have taken my name as 'Brijesh R Chaudhary'.
Now i want to change my name from 'Brijesh' to 'Brijesh Kasana'
I have following queries:
1. How can i change my name as presently i am working in Gujarat in a Public Sector Undertaking firm and my domicile is of Punjab.is it possible to change name in gujarat itself.
2. How to change my name in the LIC policy of mine and PAN card.

First you need to change your name and to publish in gazette.
You will be provided with free copies of gazette and you need to produce to LIC and PAN for change of name...
After free copies you need to get it for cost

you have to make the name change application to your state of domicile with relevant fees for the same to be published in gazette there,
along with an attested copy of the gazette notification,make a request to LIC for change of name in the policy documents and necessary correction in policy bonds.
