Affidavit for gap in cet exams?

my niece has completed 12th science in the year 2010-11, but he should apply for CET, exams but he did apply in the year 2010-11, has Gap of one year due to some reason and now institute demand to produce affidavit of one year Gap in CET exams could you provide AFFIDAVIT FORMAT FOR ONE YEAR GAP FOR CET IN TEH YEAR 2011-2012.

affidavit is statement made on oath .
you have to mention reasons why you were unable to take CET for one year what was reason for gap .
format is
