Dear Experts,
In March 2009, I had opened a PPF A/c through an agent in Post office. But I forget to take the receipt from him, Last year when I tried to contact him then I come to know that he has died in 2011. Now I know his name & where had worked in post office. Can you please tell me the procedure through which I can know whether he has opened a PPF a/c in my name or not.

You may contact the family members of the agent and get the information where the agent used to open PPF account in post office or Bank and in which Branch. You may get the address of the Agent from Post Office. After knowing the Branch of Post office or the Bank you can easily trace your PPF account from there.

Every small savings agent of a P.O. keeps a counterfoil of prescribed claim form for commision he claims from the P.O. for opening such type of accounts. So, his family must be having those claim counterfoils for the concerned period. You can check the particulars of your account if opened by him. Your PPF book may either be lying along with other documents of the agent with his family or can be available along with unclaimed passbooks with the Head Post Office to which you can claim. Otherwise, you may apply for duplicate passbook with the help of account opening particulars received from agent's family.

The agent submits in batches and one acknowledged copy is retained by agent.
Majority of the PO are computerized and the clerk at the window/Sub Post Master cans search you’re a/c number.
If you remember the month of the year ask them to look into the register.
