Registration of muslim marriage in punjab

I am a Sunni Muslim boy resided in Nabha District Patiala Punjab & married with Hindu Girl. After marriage the girl changed his name & religion from Geeta Bhardwaz to Geeta Bhadwaz Khan. Now I want to got register my marriage Under Punjab Marriage registration Act. Will you please guide me as to whether the Muslim Marriages are registered in D.C. office or Tehsildar Office. What is the procedure & requirement for registration, whether the change of name is required to be published in News paper or not
