Hopefully you have submitted details correctly to IT department while requesting pan card.
if you are right, then go to the neareast PAN facilitiation center and ask them to correct the document . show them the proof.
no Gazette required if IT department had done a mistake.

sir, by some mistake made by my lawyer the form was filled up incorrectly and there is no mistake of the income tax department. they have issued me a letter to get a public gazette notification
