hello sir/madam,
I was born on 26-09-1990 in mission hospital,kolar. At the time i was born, we were given a certificate with no name in it. Now i want a birth certificate with my name it...what is the procedure have i to follow inorder to obtain it......at present i live in hassan, karnataka. please do the needful by replying.

You should contact The Registrar of Births and Deaths,Hassan ,Karnataka. Firstly , confirm that your birth has been registered,then apply for putting in your name by filing an affidavit and get the needed certificate.

You should contact the Registar of Births and Deaths,Hassan ,Karnataka. And have to show the certificate that have been given to you by the hospital at the time of your birth.

Approach the Municipal office where you born and show the certificate that have been given to you by the hospital at the time of your birth.
