In both birth and passport name is correct. However in high school they have added fathers name. 1)How to reconcile? 2)Is there any civil/criminal liability for having different names in different documents. 3) If YES what is it?

Ans. Go for a name correction in official gazzette.
2)Is there any civil/criminal liability for having different names in different documents.
Ans. As the mistake is unintentional there is no criminal/civil liability.
3) If YES what is it?
Ans. Not applicable.

Mr. Vishal is absolutely right. no criminal liability arises if the act is not intentional. However, i am doubtful if the name can be corrected in the High School certificate. If it is a certificate issued by Board, then the Board may not permit the change to be effected in its certificate.

Palnitkar Sir is absolutely right, name correction cannot be effected in educational certificates atleast in A.P and some other states, however, in Tamil Nadu & Kerala such correction is permitted. Where such correction is not permitted the only option is to attach a certified copy of the gazzette with the educational certificates.

I think there is no issues in your name. It is common mistake / practice to add the father's name. It wont affect in any way.
