Dear Experts,

Q1: What are the requirements of an affidavit?

Q2:Is there any particular format to write an affidavit? Would you please let me how to write an affidavit for changing name of a person.

there is no prescribed format for an affidavit for changing name.
Affidavit must contain the reason for changing the name. It is sufficient, rest of an affidavit contents are as usual.
