Sbi acoount debited twice under single atm transaction

I HAD WITHDRAWN RS 1500 FROM SBI ATM. BUT WHEN I CHECKED MY ACCOUNT ONLINE, SURPRISINGLY I NOTICED THAT RS 1500 HAS BEEN DEBITED TWICE UNDER SINGLE ATM TRABSACTION NO. 9689. I HAVE MADE MANY CORRESPONDENCES WITH BANK BRANCH, DEPUTY GENERAL MANAGER, CUSTOMER CARE SBI, RBI KOLKATA BANKING OMBUDSMAN. BUT STILL MY ACCOUNT HAS NOT BEEN CREDITED WITH RS 1500. WHAT LEGAL STEPS MAY BE TAKEN TO RECOVER MY LOST MONEY WHICH HAS BEEN DIGESTED BY SBI DUE TO THEIR FAULTY COMPUTER NETWORKING SYSTEM.
please suggest how shall I write to consumer Forum ? I shall be grateful if you post a format of writing for me.

issue legal notice to SBi . move consumer forum for deficency in service and ask for compensation .

issue legal notice to them and file a consumer complaint in the consumer court. You can claim damages for not rendering better service also.

File a complaint before consumer forum and claim Rs 1500/ debited twice wrongly by Bank and also claim compensation towards mental and physical agony along with cost of complaint.
