Can anyone provide affidavit format for submission of ST-2 registration certificate to service tax dept?
After the introduction of Reverse Service tax from July 2012 we had to amend our service tax registration certificate to include services under which the service receiver had to pay service tax under reverse service tax.
So when i went to service tax dept, the officer told me to submit the amended service tax registration application also told me to surrender the original ST-2 Certificate. The problem is that we are not able to find the original ST-2 certificate. The service tax officer has told us to make affidavit that as soon as we find the original ST-2 certificate we we will surrender the same.He also told that if you say that ST-2 is lost than you will have to make police complaint and other formalities.
So can you please give format of affidavit saying that once the certificate is found we will submit the same to service tax dept.
