Mother alias name printed in my birth certificate

Hi,
In my birth certifcate my mother Alias Name(Kumari) is printed instead of original name(Sebastina). Now when i applied Passport i am facing the issue for mother name Mismatch. I am not having any record for Ailas Name(Kumari). Please kindly let me know the possible way to get the issue solved.

you can change ur mother name via gazette publication, so you contact your area gazette office, they will help you. very simple work.

Why not mention Kumari in ur application for passport. If u want to get hte certificate corrected, it would take a long time and costs of the procedure to be followed. Alternatively u may consider mentioning both the names - One as the original name and the other as alias.

@gupta Sir, where i need to mention Kumari in my passport application. and also please let me know, if i mention both names in my passport whether it will make any issues while applying for visas(H1B).

@Sheik: already i am having gazette publication, but in passport office they rejected my application and said me to bring affidavit from 1st class majistrate.

No need for Magistrate Signature, you can publish in famous News paper, that is enough.

Get an affidavit sworn by your mother declaring that her real name is Sebastina with an alias name Kumari and also declaring that in her birth certificate her alias name Kumari is mentioned. Get it sworn before a Judicial Magistrate as required by the passport Office.
